We are sending this letter to you as part of our continuing commitment to your privacy. On January 12, 2023 we became aware that your private information may have been compromised, including your name, dates of services and types of services you received at Park Place Medical Spa.
The breach occurred on January 12, 2023 and was the result of an inadvertent electronic transfer of private information to our Certified Public Accountant’s office.
We have taken a number of steps to investigate this breach and prevent any potential harm to you including providing additional HIPAA training for all employees.
We sincerely apologize and regret that this situation has occurred. Park Place Medical Spa is committed to providing quality care, including protecting your personal information, and we want to assure you that we have policies and procedures in place to protect your privacy.
